The tray fee was billed by a physician who did not perform the main procedure.

How to Fix It
Follow these steps in order to resolve the error.
- Step 1: Identify the main procedure fee code on the claim for this date of service.
- Step 2: Confirm if the physician billing the tray fee is the same physician who billed for the main procedure.
- Step 3: If your physician was an assistant or did not perform the primary procedure, you cannot bill for the tray. Remove the tray fee item from your claim and resubmit for only the services your physician provided.
- Step 4: If your physician DID perform the primary procedure, ensure both the procedure fee and the tray fee are on the same claim under their practitioner number. Correct any data entry errors and resubmit the claim.
- Step 5: If you have verified that your physician was the proceduralist and the corrected claim is still rejected, contact the Teleplan Service Desk at 604-683-7151 to investigate the claim processing logic.
