Invalid Tray Fee Location

The tray fee was billed for a service performed in a hospital or facility, where it is not a payable benefit.

Official Description: Tray fee not payable to hospitals or extended care facilities, etc.

Follow these steps in order to resolve the error.

Verify the Location of Service (LOS) code submitted with the claim. Tray fees (e.g., fee item 00010) are only payable for procedures performed in a private physician's office.
If the service was correctly billed as being performed in a hospital (in-patient, out-patient, emergency) or an extended care facility, the tray fee is not payable. The cost is covered by the facility's budget.
Remove the tray fee from the claim and resubmit it with only the payable service fees. This portion of the original claim cannot be recovered.
If the Location of Service was entered incorrectly and the procedure was actually performed in your office, correct the LOS code on the claim and resubmit.
