Multiple Services on One Line

The claim was rejected because more than one fee item was entered on a single service line.

Official Description: Each service must be on a separate line.

How to Fix It

Follow these steps in order to resolve the error.

  1. Locate the rejected claim in your billing software.
  2. Examine the service lines for the date of service in question. You have entered multiple fee items on a single line.
  3. Delete the incorrect multi-code line item.
  4. Create a new, separate line item for each individual service provided. Ensure each line has the correct fee item, date of service, and diagnostic code.
  5. Save the corrected claim and resubmit it to Teleplan.
