The claim was rejected because more than one fee item was entered on a single service line.

How to Fix It
Follow these steps in order to resolve the error.
- Locate the rejected claim in your billing software.
- Examine the service lines for the date of service in question. You have entered multiple fee items on a single line.
- Delete the incorrect multi-code line item.
- Create a new, separate line item for each individual service provided. Ensure each line has the correct fee item, date of service, and diagnostic code.
- Save the corrected claim and resubmit it to Teleplan.
