You submitted text in both the claim comment and the note record; Teleplan only accepts one.

How to Fix It

Follow these steps in order to resolve the error.

  1. Locate the rejected claim in your billing software.
  2. Review the information you entered in both the 'Claim Comment' field and the 'Note Record'.
  3. Combine all necessary text into one field. Use the 'Note Record' for detailed explanations or the 'Claim Comment' for brief, standard remarks.
  4. Delete all text from the field you have chosen not to use, leaving it completely empty.
  5. Save and resubmit the corrected claim to Teleplan.
